An EPC or CDM is a care plan provided by a GP to patients with chronic conditions such as diabetes, kidney disease and musculoskeletal conditions. It allows patients to receive 5 free or subsidised treatments per year with an approved specialist.
To be eligible you must first see your GP.
When your GP assigns you an EPC or CDM, they will usually recommend a particular specialist. You, however, can choose any specialist, as long as they are Medicare-accredited like iSpine’s practitioners.
